I hereby fully grant permission to Kim Lewis Photography to use my likeness in a photograph or other digital reproduction in any and all of its publications, including website and blog entries, without payment or any other consideration.

I understand and agree that these materials will become the property of Kim Lewis Photography. I hereby irrevocably authorize Kim Lewis Photography to edit, use, copy, exhibit, publish, distribute, and reproduce these photos for any lawful purpose.

I waive all rights of privacy or compensation or to inspect and approve the finished product, including written or electronic copy, in which my likeness appears.

Kim Lewis Photography will remain owner of all copyright but grants permission to the customer named below to reproduce the copyrighted photos specified here. The photos can be reproduced in the original only. Any additional editing (cropping, changing to black and white, etc.) is strictly prohibited in order to maintain the integrity of the work. It is also in violation of Federal copyright law. The copyright is being released on all photos taken for customer on the specified photo shoot date.
